wellness coordinator

*Eden Villa Retirement & Assisted Living* is a one level 44-bed home located close to downtown Chatham. The Wellness Coordinator is responsible for developing and maintaining social recreational, health and educational programs and activities for seniors in both group and one-on-one.

*RESPONSIBILITIES*

* Work collaboratively with the Resident Care Manager, Assistant Care Manager and other members of the Eden Villa care team
* Actively participate in Falls Prevention and staff meetings pertaining to their role
* Work under the facilities policies and procedures
* Meet regularly with Resident Care Manager and General Manager to discuss individual, team, and program effectiveness. Provide input into Eden Villa recreation policies
* Work collaboratively with external care providers (PT/OT) following nurse rehab directions for individuals
* Focus on *mental health, physical wellbeing, workplace culture*,
* Other tasks as assigned

*QUALIFICATIONS:*

* Programming experience
* Experience in Falls Prevention and Nurse rehab
* Current GPA (Gentle Persuasive Approach training) or equivalent
* Good organizational skills
* Outgoing and compassionate skill set
* Ability to work in a team environment as well as independently; self starter
* Able to communicate verbally and in written form
* PTA (Physiotherapist Assistant), PSW, Gerontology or related academic experience (i.e., activation coordinator) required
* Experience in creating and maintaining individual Care Plans; POC/PCC electronic documentation experience an asset

Able to provide a satisfactory Vulnerable Sector Police Check.
