*Apartment Registered Nurse (ONA) *

Reporting to the Manager, Assisted Living and Community Programs, the Apartment RN will support the needs of the Assisted Living residents. To ensure that residents of the Assisted Living Program have their healthcare needs met and monitored in a manner consistent with Shalom Community Village ATHOME values. The Apartment RN uses advanced training to complete resident assessments, create care plans, and ensure that residents are encouraged to be independent with their care needs.

1. *Resident Care Management*

* Ensure resident-centered care and support is a priority in care management.
* Ensure resident choice is considered in care management.
* Conduct comprehensive assessments of residents? health status, including physical, mental, and psychosocial needs
* Develop individualized care plans. Initiate care plan on admission, annually and when a resident care needs change
* Co-ordinate with the Manager of Assisted Living program to review and ensure that resident needs are being met
* Collaborate with Shalom Village team members to promote resident wellness.
* Respond to emergencies and initiate appropriate protocols
* Monitor and administer medications, and treatments.
* Collaborate with the resident served, treating physicians, members of family/support system and other stakeholders are involved to promote resident wellness.
* Initiate and assist Pharmacy teams with seasonal Vaccine Clinics for resident
* Attend and present nursing updates at monthly resident town halls.
* Educate residents and families on health conditions, medications, and care strategies.
* Initiate resident care conferences with other stakeholders as required

2. *?Documentation and Compliance*

* Maintain accurate records of assessments, treatments, and medication monitoring and administration.
* Ensure compliance with provincial regulations, College of Nurses, and Shalom Community Village policies.
* Record on progress notes, assessments, incidents on Point Click Care
* Ensure that all documentation complies with regulatory requirements and Shalom policies. Charting method is SBAR

3. *Care Planning and Direct Care:?*

* Complete RAI-Cha assessments as needed.
* Discuss and review DNR orders with residents, and update the team on status
* Ensure medications are delivered intact and checked on arrival from pharmacy
* Follow up with pharmacy on all medication changes
* Ensure PSWs monitor the residents' medications according to prescribed dosages and schedules, as well as monitoring for adverse reactions or interactions.
* Administer injections for residents on (q 6-month Prolia/Jubbonti) shots,
* Change diabetic sensors weekly
* Organize and prepare weekly blood work
* Ensure PRN policies are adhered to, and PSWs have knowledge of the medications
* Collaborate with PSWs and community wound care nurses to manage residents? wounds, including pressure ulcers, surgical incisions, and skin tears.
* Collaborate with PSWs to monitor residents? vital signs, symptoms, and changes in condition, reporting any concerns or changes as needed.

4. *Quality Assurance:*

* Coordinate with Manager of Assisted Living, monthly data collection on resident well-being and falls activity, input stats into PCC (QHI dashboard)
* Identify and create action plans to ensure resident risk is reduced
* Respond to resident concerns and complaints, escalate to Manager of Assisted Living as needed
* Assist Manager of Assisted Living with monthly audits of progress notes, point of care entries, and reports with follow up action items
