About the Role

*Overview*
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and resources to children and families in need. This role requires a strong understanding of case management, special education, and pediatric care. The Social Worker may collaborate with the team to ensure comprehensive patient care and advocate for the best interests of our clients.

The Wellness team at Niagara Naturopathic & Wellness Clinic includes clinic owner and Naturopathic doctor Jacquie Moulton, along with a Psychotherapist, Homeopathic consultant, Kinesiologist, & Hypnotherapist. Note this is an independent contractor position with flat fee rental preference however we can negotiate a split situation.

*Duties*
* Conduct assessments to determine the needs of children and families.
* Develop and implement individualized care plans based on assessment findings.
* Provide case management services, coordinating with healthcare providers, educators, and community resources.
* Assist families in navigating special education services and accessing necessary support.
* Utilize medical terminology to communicate effectively with healthcare professionals.
* Engage in data collection and research to improve service delivery and outcomes.
* Maintain accurate documentation of client interactions and progress.
* Advocate for patients rights and facilitate access to appropriate resources.
* Participate in interdisciplinary team meetings to discuss patient care strategies.

*Qualifications*
* Bachelor?s degree in Social Work or a related field; Master?s degree preferred.
* Experience working with children in a clinical setting is highly desirable.
* Knowledge of special education processes and pediatric care practices.
* Strong skills in case management, including assessment, planning, implementation, and evaluation.
* Familiarity with medical terminology relevant to patient care.
* Excellent communication skills, both verbal and written.
* Ability to conduct research and analyze data effectively.
* Compassionate demeanor with a commitment to serving diverse populations.
* Strong organizational skills with attention to detail.
